Eastern Ontario MRI Central Intake Program

Central Intake for MRI has been established in our region to improve coordinated access for semi-urgent and non-urgent MRI scans. The program aims to simplify the intake process for referrals and improve equity in access to care. Referrals sent to MRI are fed to  organizations across Eastern Ontario to efficiently match patient needs with available resources.

Champlain GI Endoscopy Central Intake

 

Champlain GI Endoscopy Central Intake was established in 2022 to address the backlog of cases that resulted from the pandemic and enhance patient access to endoscopies. Pathways currently supported by Central Intake include cancer-related endoscopies for Abnormal FIT, Screening for those with family history of cancer, and surveillance for those with previous polyp removal. The program is currently available in Ottawa (The Ottawa Hospital, Queensway Carleton Hospital, Montfort Hospital), Hawkesbury (Hawkesbury and District General Hospital), Cornwall (Cornwall Community Hospital), Glengarry (Glengarry Memorial Hospital), Pembroke (Pembroke Regional Hospital) and Renfrew (Renfrew Victoria Hospital).

Rapid Access Clinics for Low Back Pain

The RAC Low Back Pain program (formerly known as ISAEC) is an innovative, upstream, shared-care model of care in which patients receive rapid low back pain assessment (less than four weeks on average), education and evidence-based self-management plans. It is designed to decrease the prevalence of unmanageable chronic low back pain, reduce unnecessary diagnostic imaging as well as unnecessary specialist referral. Referral into the program is only available to patients whose family doctor or primary care nurse practitioner has enrolled in the program.

Champlain MSK Care

 

Champlain MSK Care runs two central Intake programs for Orthopedic services in the Champlain region: Hip and Knee Rapid Access Clinics, and the Champlain MSK Central Intake Program.


The Hip and Knee Rapid Access Clinic (RAC) – formerly called the Total Joint Assessment Clinic (TJAC) – is a provincial healthcare initiative that was started to provide all Ontarians with timely access to high-quality, integrated musculoskeletal (MSK) care for joint pain and disability related to hip and knee osteoarthritis. Patients referred to the program will be assessed by a specially trained Assessor (nurse or physiotherapist) who will perform a standardized assessment and will discuss care options and make recommendations for care. If the patient meets the criteria for surgery, they will be referred to an orthopedic surgeon who will meet with them and decide if surgery is the best option.

Champlain's MSK Central Intake Program supports multiple referral pathways. Patients may be referred for surgeon consults for issues relating to Shoulder, Foot and Ankle, Knee Conditions and Spine. These high volume orthopedic referrals are funneled into one place so that referrals can be directed to the right surgeon and/or Rapid Access Clinic as quickly as possible and in an equitable, fair manner. The MSK program includes the Foot and Ankle Screening and Triage Clinics at QCH, TOH and Montfort Hospital.
